Complete the Application Form on "2020 Canadian Chinese Song-Writers Quest Singing Contest (SQSC)" official websites (www.am1470.com / www.fm961.com). 登入「SQSC 聲動溫哥華歌唱大賽」大會網頁並填妥網上報名表格。
Upload the Photo (jpg) of the Contestant and provide a link (e.g. Youtube unlisted, Dropbox or Google Drive) to the 1-min Video. 上載參賽者照片(jpg) 並提供參賽者唱歌短片之連結（例如 Youtube unlisted、Dropbox 或 Google Drive）。
After submitting the Application Form, you will be assigned a reference number. After you log out, a confirmation email will be sent to you. 遞交表格後，若成功報名，你將會獲發編號及收到確認電郵通知書。

Apply by mail or in person 郵寄或親身報名

Drop-off or mail the completed Application Form, the Video (on a DVD or USB) and the Photo of Contestant to Fairchild Radio (Unit 2090-4151 Hazelbridge Way, Richmond, B.C., V6X 4J7). 填妥及簽署報名表格，連同參賽者唱歌短片 (DVD 或 USB) 及參賽者照片，一併郵寄或遞交至加拿大中文電台溫哥華辦事處（Unit 2090-4151 Hazelbridge Way, Richmond, B.C., V6X 4J7)。

Fairchild Radio shall not be held responsible if the Contestant’s entry or application cannot reach Fairchild Radio before deadline due to technical reasons such as internet performance or postal delay. 如因網絡線路繁忙、郵遞延誤或其他技術原因，而令參賽者之電郵或表格未能在截止日期前傳送到加拿大中文電台，本台概不負責。

Contestant’s Declaration 參賽者聲明 :

By completing this Application Form and submitting same together with the Video and Photo, all Members listed in the Application Form (collectively called the“Contestant”) hereby enter into a binding agreement with the Organizer FAIRCHILD RADIO GROUP LTD. (hereinafter called “Fairchild” ) and make the following representations, warranties and covenants. 報名表格中列出之所有成員（共成一個個體稱為「參賽者」），於填妥此參加表格、連同參賽歌曲、短片及照片遞交之同時，已即時與主辦機構加拿大中文電台（以下稱「加拿大中文電台」）簽訂具約束力之合約，並作出以下聲明、保証及承諾：
The Contestant must be Canadian Citizen, Permanent Resident or Landed Immigrant of Canada , and is not an employee, nor the immediate family of an employee, of Fairchild or the co-organizer(s) of the " 2020 Canadian Chinese Song-Writers Quest Singing Contest(SQSC) " (“ the Event”) 參賽者均為加拿大公民、永久居民或已抵埠移民，非加拿大中文電台或「SQSC 聲動溫哥華歌唱大賽」協辦機構之員工或員工家屬。
All items submitted are non-returnable. 所有遞交的資料一概不予發還。
The Contestant assigns all rights and copyrights of the Video to Fairchild for promotional purpose without any further compensation whatsoever. 參賽者同意將參賽者唱歌短片之版權及所有權利無條件授予加拿大中文電台作宣傳之用。
The Contestant confirms the information provided in the Application Form is true and accurate. 參賽者確定參加表格內所提供之資料真實及正確。
Contestant has no legal obligation that would prevent the Contestant’s participation in the Event or compliance with the terms and conditions hereafter set forth. 參加者並未受任何法例所限制而導致其不能參加「SQSC 聲動溫哥華歌唱大賽」，或不能遵從相關之所有參賽規則及條例。
Contestant recognizes that the words "Canadian Chinese Song-Writers Quest Singing Contest (SQSC)" are the property of Fairchild, and the Contestant shall not ever use, or authorize others to use, the words " Canadian Chinese Song-Writers Quest Singing Contest (SQSC)" for commercial or promotional purposes without the permission of Fairchild. 參賽者確認「SQSC 聲動溫哥華歌唱大賽」之名稱為加拿大中文電台所擁有，在未經加拿大中文電台同意下，參賽者不能利用或授權他人利用「SQSC 聲動溫哥華歌唱大賽」之名稱，從而作商業或推廣用途。
If the Contestant is selected as one of the finalists, the Contestant will be required to sign a “Finalist’s Agreement” with Fairchild, and will agree to observe all rules and regulations in regards to the Event such as staying in town to participate in training and rehearsal, and complying with image consultation. In the event the Contestant requires live music performance during the Final Contest, approval from Fairchild is required. 若被獲選晉身決賽，參賽者必須與加拿大中文電台簽署「決賽參賽者同意書」，並遵從加拿大中文電台所訂定之比賽條例及守則，例如留在溫哥華以參與訓練和綵排、配合形象指導，又或決賽時如需要有樂器現場伴奏，須得加拿大中文電台同意及配合等。
Refusal to sign the “Finalist’s Agreement” will result in immediate disqualification of the Contestant and no further participation in the Event without any liability on Fairchild. 若晉身決賽之參賽者未能簽署「決賽參賽者同意書」，參賽者將被即時取消資格，並喪失所有「SQSC 聲動溫哥華歌唱大賽」之參與權利，加拿大中文電台無須為此而負上任何責任。
If any of the above representations, warranties and covenants made by the Contestant is false, Fairchild reserves its right to disqualify the Contestant without any liability on Fairchild. 若參賽者於以上所作出之聲明、保証及承諾有不實或誤導之成分，加拿大中文電台有權取消參賽者之參賽資格，且無須為此負上任何責任。
If there is any inconsistency or ambiguity between the English version and the Chinese version, the English version shall prevail. 本文以英文及中文撰寫，如有抵觸，以英文版本為準。

Fairchild Radio shall retain the Contestant’s information for the purpose of coordinating the “Canadian Chinese Song-Writers Quest Singing Contest (SQSC) ”. All information shall be kept confidential and destroyed when the Event is completed. 加拿大中文電台將保存參賽者資料作「SQSC 聲動溫哥華歌唱大賽」之用。所有資料將絕對保密並於比賽結束後銷毀。
